The report of findings is a straightforward summary of the case history and examination findings. The diagnosis of your condition is explained to you and a treatment plan is prescibed. The “Report of Findings” will identify the most appropriate treatment, the estimated number of treatment visits required for your condition, and a review evaluation date will be given.
